Allergy Testing

There are risks and complications associated with allergy testing. These include, but are not limited to: Local reactions consisting of swelling, itching, pain, burning. Etc; Infection (rare); Anaphylaxis (rare); Asthma attack I have had the opportunity to have all of my questions answered to my satisfaction. I fully understand the benefits, risks, complications, and alternatives to being allergy tested. To the best of my knowledge I am not pregnant. I consent to being allergy tested.

Allergy Shots

Allergy shots are a weekly injection immunotherapy. The purpose of this therapy is to gradually introduce the substances that you are allergic to into your immune system each week. Over the course of a few years, this therapy will desensitize your immune system to these substances. Your immune system will no longer think of these things as foreign invaders and will stop over-reacting with undesirable symptoms. This process involves coming into the office once per week, every week for approximately 2-3 years. Insurance will usually cover the cost of this therapy with a copay at each shot visit. Due to safety concerns a doctor must be present in the building when injections are administered so we suggest calling or verifying availability online prior to coming in. By electing to pursue this treatment the patient accepts financial liability for the costs related to making the allergy serums. It is the patient’s responsibility to check coverage for this service with their insurance company directly.

Should the patient elect to discontinue Allergy Shot treatments, the charges for serums already produced will be billed to the patient’s insurance company. As this is a highly personalized treatment serums cannot be used for other patients; thus, the patient assumes financial responsibility for vials which have already been prepared.

Sublingual Immunotherapy or SLIT

SLIT is an oral allergy desensitization therapy which is currently not covered by insurance. By electing to pursue this treatment patients accept financial liability for the costs related to the allergy drops. Patients with flexible spending accounts may opt to pay using their benefit card.

Prices are guaranteed for 1 year. Due to rising costs of supplies, a price increase may become necessary. Lakeside ENT & Allergy, LLC will notify patients in writing if a price increase is taking place.

We offer 2 plans that you can choose from. Each treatment can be paid for by Credit Card, Debit Card, Personal Check, Flex Spending Accounts, or you can apply for Care Credit – www.carecredit.com

PLAN 1- Patients may pay for the 1-year treatment up front for $900.00. You will receive 1-year worth of drops (4 bottles) and receive a discount for paying upfront.

PLAN 2 – Patients can also pay for each bottle. The payment for each bottle is $250.00. *The patient is responsible to call the office when another bottle is needed. Please allow at least 1 week for prep of a new bottle. Each bottle is made to last 3 months – The payments for 1 year of treatment add up to $1000.

**If extra bottles are needed because of lost bottles or misuse, the price for each bottle is $250.00.

Because this treatment is a personalized care plan, the patient is financially responsible for any prepared bottles.

Treatment bottles will not be mixed until payment has been made.
